Clarke County · Kemper County · Lauderdale County · Wayne County Jury Summons Online Excuse Affidavit If you are over 65 years of age and do not wish to serve, you have a statutory exemption. Therefore, please so state on the affidavit and e-mail the Court Administrator immediately. However, you may serve if you desire. If you have served on a petit jury panel in the Circuit Court or County Court in an actual trial within two (2) years and do not wish serve you have a statutory exemption. Therefore, please provide the dates upon which you served on the affidavit and e-mail the Court Administrator immediately. Once this information has been verified you will receive an email from the Court Administrator. However, you may serve if you desire. If you claim an exemption because of illness please complete the online affidavit below and fax (601-486-4933) or e-mail (dwedgworth@lauderdalecounty.org) the doctor’s excuse to the Court Administrator immediately. You will receive an e-mail confirmation as to whether you are excused.
If your attendance would cause
serious financial loss to yourself or your business, please e-mail the
affidavit explaining this to the Court Administrator immediately. Please
mention in the affidavit the type of business you work in, and why your
absence will cause a financial hardship. If you are not
notified that you are excused, then you are expected to appear at the time
indicated on your summons or as per recorded instructions.
If you claim any other excuse
because of emergency or hardship to yourself, please state your reasons on
the affidavit and e-mail the Court Administrator immediately. You will
receive an e-mail as to whether you are excused. If you are not notified
that you are excused, then you are expected to appear at the time indicated
on your summons or as per recorded instructions. If you are under the age of twenty-one (21), or are unable to read or write, or are a convicted felon please indicate on the affidavit and e-mail the Court Administrator immediately. Once this has been verified, you will receive an e-mail as to whether you are excused. Names are randomly selected from the County voter rolls. If you are no longer a resident of the County and have received a jury summons, you will need to contact the Circuit Clerk’s office to have your name removed from the voter roll. AFFIDAVIT
